Nominations for Assistant Coach of the Year, Junior High/Middle School Coach of the Year, or Member of the Media, please write:

1. Name of your nominee,
2. The award you’re nominating for,
3. Their school or organization,
4. A brief paragraph explaining why you think this individual should receive the award, and
5. Your name and school.
